According to Kouta, from 0:05.4 to approx 0:09 is a reused Tomioka scene from episode one. 0:15.4 - 0:23.5 is probably Kouta but genga for that part is not shown in his book. From 0:28.6 - end it's someone else.

Kouta also mentioned that he learned a lot from the NC effects AD, who is most probably Kaichirou Terada
